At Test Registration Time

Students registering to take the ACT may list in the score recipient box up to four colleges/institutions to receive their profile report. Include Ferris' college code number 1994 in this list.

Additional Score Report Requests

Students who have taken the ACT and did not list Ferris as a recipient of the profile report, must initiate a score report request to ACT. To do so, contact ACT at the national office in Iowa City by one of these methods:

1. Mail - Send a request to ACT, P.O. Box 168, Iowa City, IA 52240.
2. Phone - Call ACT's Records Department at 1-319-337-1313 from 8 a.m. until 5 p.m. Central time.
3. On-line - Go to ACT's Web site at www.act.org/aap/scores/index.html. Complete and submit the request form.
